Physicians and other health care providers in the following California counties are eligible for membership in PFMC: Alameda, Butte, Colusa, Contra Costa, El Dorado, Glenn, Imperial, Lassen, Marin, Modoc, Napa, Nevada, Placer, Plumas, Sacramento, San Diego, San Francisco, Shasta, Sierra, Siskiyou, Solano, Sonoma, Tehama, Trinity, Yolo, and Yuba. Providers in other California counties can join similar networks run by their local medical foundations. For a complete list of foundations, see our PFMC Service Area map.

To apply for PFMC membership, simply download an application form or request one from us at the address below. Then return the completed form via fax or mail. Our staff handles all credentialing, saving you the trouble of credentialing with multiple payers. Completed applications are submitted to our physician-led board of trustees for final approval. The entire process can usually be completed in a matter of weeks.
